West Florida Health Network (WFHN) is a clinically integrated network serving an exclusive health population in Hillsborough, Pinellas and Pasco counties. Our physician partners work together to provide the highest level of care to our patients through open communication, population health tools, care management support and educational opportunities. As a Clinically Integrated Network, WFHN partners with high performing physicians who are committed to exceeding quality benchmarks. The network includes providers across nearly every discipline. Our population health team focuses on expense reduction as it pertains to utilization and quality metrics. Our care management team is dedicated to managing patients with chronic disease and improving health outcomes.

    As a patient as well as a professional in the healthcare field, you understand the importance of taking an active role in your medical care. West Florida Health Network was created to empower you to be an equal partner with your physician in creating an individualized plan of care to specifically meet your unique needs.   • Network Physicians

    As a Clinically Integrated Network (CIN), we provide the vehicle and create the infrastructure to aide our providers in transitioning from a fee-for-service care model to one that is value-based. This is accomplished through collaborative efforts between the network’s hospitals and providers to proactively manage their patients’ care by focusing on population health management, preventative care and patient education.     The WFHN goals include improving quality of care and clinical outcomes, improving coordination and continuity of care, improving efficiency of care, eliminating unnecessary and persistent clinical care variation, applying evidence-based medical interventions, and supporting comprehensive clinical care with use of an integrated clinical information technology platform. WFHN also will negotiate new contracts with payers on behalf of its participating physicians based on their ability to achieve established clinical performance standards and benchmarked milestones.

    At West Florida Health (WFHN), we recognize that budgeting for employee healthcare can get expensive. West Florida Health Network was created as a vehicle to reduce the health care costs while improving the quality of care provided for the employees of Tampa General and Florida Hospitals. WFHN is now able to offer these healthcare services – an extensive provider network, nurse care management for at-risk members and claims support – for local self-funded employer groups.
